How to Make an Incident Report
As we said, incidents happen now and then. And every company leader must be prepared when dealing with such instances. Create incident reports to help document and address any accident that may occur in the company. Incident reports need to be concise and straightforward. It is okay if the content is not lengthy as long as the message gets relayed well. We prepared a few steps to help you out in the process.
Step 1: Recount the Incident that Happened
Stop and think for a minute. Recall the incident that happened and try to remember the details that might be relevant in your report. You can even go to the area where the incident occurred and look around. Use any bit of information in the area that is relevant. Just make sure that it was a part of the event and don’t put it, so you have a piece of evidence.
Step 2: Ask Other Witnesses About the Incident
If you found no satisfying piece of evidence for the status report, you can input in crucial witnesses that were present during the time the incident occurred. Ask multiple key witnesses about what they saw during the event. You can ask about the time, the person involved in the scenario, the items used, and many more. List all of the information down in an orderly manner.
Step 3: Assess the Validity of the Information

Step 4: Create an Initial Draft of the Report

See if there are any contextual or grammatical mistakes that you might have missed. Take note of these and edit them one by one. Do I need to call the cops for any incident that happens?
It highly depends on the situation. If an incident happens inside a company, then you need not involved the cops unless there are robbery or something similar is involved within the building or vicinity. But, usually, you can only call the cops when it is an outside-of-the-company kind of incident.
Is my report still valid even if I don’t cite any other witnesses?
The incident report can still be accepted, but it won’t be as valid as you might think. Without further proof of the incident, the higher management may doubt the report’s claims and details of what happened. So as much as possible, do cite other witnesses on the incident report, and you must have them sign the document. If there are no other witnesses, you must gather as much proof as possible.
Can incident reports be used in court?
Yes. At some point, your incident report can become a vital piece of evidence in a court. And so, you must mind what personal information you will include in your incident report. Medical information and many more like it should not be in the report as it might also be used against you in the court by the one you reported. For example, if you include that you have a mental problem, the judge might think you made it all up, and he might even discredit its validity.
