Whether you are student thinking of going to nursing school or an experienced nurse practitioner, you need to be able to effectively fill out a nurse application form when the necessity arises. Even if its just a simple application form, the amount of information you write on it may make or break your application.

Enter accurate personal data. Just like filling out any type of application form, you need to always write accurate personal information like name, date of birth, age, marital status, race/nationality, name of school attended, the date your nursing degree was completed, etc. These basic personal information is used to differentiate you from other applicants and also serves as a way of record-keeping.
Obtain academic record from past schools or universities attended. Applying for nurse positions will often require you to submit past academic records. Some employers may need this for documentary or verification purposes. For those who want to go to nursing school, scholastic records are needed to help determine if a student can meet the school’s academic requirements. Some schools may only admit students who meet a certain GPA score.
Provide an accurate list of past training programs attended. Graduate nurses often still require additional training and certification on phlebotomy and first aid. Other training programs will be needed depending on the specialization field you go into.
When filling out a nurse application form, it is better to list all the nurse training courses that you have completed than to leave out the training programs that you deem unimportant. Having a wide variety of completed training programs gives employers the impression that you are a well-rounded individual and may be fit enough for job.
