What is a Pre-Employment Physical?
A pre-employment physical is one of the company’s requirements from potential employees. These are physical examinations and health inquires which includes alcohol and drug tests, psychological tests, and physical and mental health assessments.
These exams have to do with the nature of the job and is essential especially for those who do field works. The use of pre-employment physical examinations is not just to protect the employees from injuries at work, but also to protect the company against work-related lawsuits for injuries that could have been prevented before the employee gets qualified for the job.

A pre-employment physical exam starts with a general physical exam. In the general physical exam, the vital signs and senses will be checked whether they are working properly. During this first step, the medical history will also be reviewed and documented. Some employees would be required to have a respiratory evaluation to check lung capacity.
This is essential most especially if the job requires the wearing of the mask in order to breathe. In evaluating the respiratory functions, the heartbeat will also be checked if ever there are any problems that are suspected. An employment physical form is filled up by the physician or a doctor. This basically indicates all the results of the test that was undergone in examining the physical and mental health of an individual employee.
This examination is conducted to ensure that the employee is in good health and able to do the job that he or she applied for. Also, this examination is conducted to protect the company against liability claims. All these procedures are not or cannot be completed without the work physical form.
These are the following inclusions of an employment physical form:
- Personal data of the employee such as the name, title, department, contact details, etc.
- Doctor or the physician’s medical license, name, address, contact details
- Vaccination history or communicable diseases (include date of last examination)
- Physical examination/ Medical Evaluation/ Immunization or laboratory tests
- Review of the symptoms or summary of the findings
- Consent, name with affixed signature of the doctor or physician, and the date signed
