Types of Evaluation Forms

There are different types of employees in the typical workplace hierarchy, and all are (or should be) subject to evaluation from their coworkers.

A good evaluation form identifies clearly what is being evaluated and who is doing the evaluation. An employee may be

  • evaluating a supervisor or manager (or vice versa)
  • evaluating colleagues, or
