Evaluation of employee performance is a routine reality in the best and most quality-conscious businesses. We can all agree that it is crucial for the company’s long-term success to maintain performance – and even find opportunities where work practices and operations can be improved.

A time-tested way to go about evaluation is to distribute Evaluation Forms to employees.

Here are some free and practical evaluation forms you can use to evaluate employees. This is a great place to start, whether you need to download a ready-to-use form or a workable PDF or Word doc template.

Types of Evaluation Forms

There are different types of employees in the typical workplace hierarchy, and all are (or should be) subject to evaluation from their coworkers.

A good evaluation form identifies clearly what is being evaluated and who is doing the evaluation. An employee may be

  • evaluating a supervisor or manager (or vice versa)
  • evaluating colleagues, or
  • evaluating himself or herself.

(See Employee Review Forms and Self Evaluation Forms for more free downloads in PDF and Word doc.)

The evaluation may be general, or it may be focused on a core trait or competency relevant to the employee’s performance within the company.

Evaluations are usually also given at the end of a training course or session, or after a specified period for standard routine evaluation (see the 90-day employee evaluation form below).

Some examples offered here for free focus on employee attitude and on employee attendance.

These are useful when the eye of management must focus and review on one important aspect of their company.

Evaluation forms provide an easy way to gather information for controlling quality and maintaining performance for the company as a whole.

Incidentally, these forms should be kept confidential. Protocol may vary for your company, but generally, evaluations are kept between the HR department, the employee under question, and any other supervising manager in charge of the employee.

All are available for free and easy download.

You can print them out and start using them immediately, or you can build your own evaluation form using these practical examples as templates.

They are all intuitive and easy to use.

Since most evaluation forms require answers to close-ended questions, usually using a scale or rank, it is also easy for the evaluator to analyze responses.

These forms provide a formal, objective context – even if you are filling out an Employee Self Evaluation Form, you can expect to get an adequately accurate picture of your own performance using these workable templates.
