Difference between a Birth Certificate and a Certificate of Live Birth

A Certificate of Live Birth is the first draft of a Birth Certificate. The information in a Certificate of Live Birth is entered by the hospital personnel into their hospital database. This will include basic information, such as the baby’s name, parents’ names, doctors’ names, hospital where the baby was born, date of birth, sex, race, and the person who is completing the record. This information will then be checked by the father of the baby, since the mother might not be physically able to peruse the information, before it is sent to the Office of Vital Statistics or State Register. A Certificate of Live Birth is unofficial proof that a mother gave birth to a child.

Once the Certificate of Live Birth is entered, checked for any mistakes, completed, and sent to the Office of Vital Statistics or State Register, the official Birth Certificate is then created. While a Certificate of Live Birth is medical proof of one’s birth, a Birth Certificate is the official record of the government declaring his birth. This is the official document that would have to be recorded and presented for official purposes, such as getting a passport. This should also be printed on high-quality paper with the date and signature of a state official at the bottom and a registrar’s state seal embossed in the corner. Information Included on a Birth Certificate Form

  • Name of the newborn baby: This should be in full. No initials are allowed.
  • Date of birth: This would include the date, month, and year. There are also Birth Certificates that contain the time of birth.
  • Location of birth: This would include the specific city where the baby was born.

Dental Certificate Forms

This is used by employers and educational institutions to make sure that their employees and students, respectively, have healthy gums and teeth. This is part of assessing an individual’s oral health. Divided into Two Sections

  • One part has to be filled out by the individual or his parents or guardians, if he is a minor or child under the age of 18. This section contains the individual’s personal information, such as his name, birth date, sex, and age. For students, it would also include the name of the school or university he is attending, and what grade or year he is in. For employees, it would include his employment information, such as his department, employment number, direct head or supervisor, and job title.
  • The second part is the one that has to be filled out by the dentist or dental hygienist. This contains information on the individual’s oral health status, such as the presence of any dental sealants or open cavities. Aside from that, the dentist would also need to state if the individual needs dental treatment and to what extent the treatment will be. It would be specified if he needs urgent treatment (which includes symptoms of pain, infection, or swelling), restorative treatment, or preventive treatment.

A Dental Certificate Form attests that the individual has been checked by a dentist or dental hygienist. And at the end of the form, the dentist would have to indicate if the individual is fit in terms of dental health to be attending a school or starting a new job. A Medical Certificate Form or Health Certificate Form works the same way as a Dental Certificate Form, but there are more sections to be filled that would cover the overall health of the individual, not just his dental health. And instead of a dentist, the second section would have to be filled out by a physician.

Conduct Certificate Forms

These are used by educational institutions or other establishments to certify that a person is of good moral conduct. This will serve as the person’s character reference when applying for jobs or enrolling in a new school or university.

Gift Certificate Forms

Gift Certificate Forms are prepaid forms that can be used instead of money when getting products and services from a retail store or any related business. This can be given as gifts, hence the name, for loved ones and friends during special occasions. Gift certificates are also bought in bulk to use as prizes in big events.
