What is a Teacher Feedback Form?

Teacher feedback forms are used by just about every school for when they need to understand how students feel about particular teachers. Other teachers may also give feedback on their fellow coworkers regarding how they are able to educate all the students that are under them.

These forms can be very useful for teachers that receive the feedback because it gives them the opportunity to know what their students are unsatisfied with. Thus, giving them the chance to change certain qualities about themselves to ensure that the students are with a teacher that can provide them with the education they need.

How to Administer Teacher Feedback

If you plan on giving any sort of feedback  to a teacher, it’s best that you know which teacher you’re giving the feedback to. So you’ll have to know the complete name of that teacher, as well as the subject that the teacher instructs and the year level the teacher educates.

You will then have to state the type of feedback that you’re giving. For example, you’re an instructor that has recently finished a training session on training current and would-be teachers, and would like to use training feedback forms to assess how these teachers performed. Then you must properly rate all the aspects to show how you properly feel about a specific teacher.

How Important is a Teacher Feedback?

Teacher feedback is very important so that schools will have a better understanding as to the type of teacher that they have hired to educate their students.

A great way of knowing how these teachers are performing are through the students. Which is why there are forms such as student feedback forms which allows students the opportunity to place in how they feel about the different qualities and teaching methods a certain instructor brings.

There are also other forms that contribute as to how certain students feel about a particular teacher. A good example would be through the use of course feedback forms where not only does it show if the course was beneficial to the student, but also if the teacher was able to properly hand out the knowledge. A teacher may be replaced if all the students feel unsatisfied with the teaching methods, so it’s best that these teachers do their best to get positive feedback.

How to Write a Feedback Letter to a Teacher?

If you plan on writing any feedback letter to your teacher, then it’s best to take note of the following information:

  • Complete name of the teacher and that department that he or she is designated in
  • Your complete name as well as your student ID number and year level
  • Paragraphs regarding the different aspects of the teacher that you are happy or unhappy with
  • The date of when the letter is issued
  • Your signature

Once you start writing the feedback letter, always be sure to be as respectful as possible. Think of it in the same sense as issuing parent feedback forms in which you have to show courtesy regarding how certain parents are handling their children.

It also helps to be specific on what you would like to give feedback to and present arguments as to why you feel that way. Doing so will allow the teacher to know full well about what he or she needs to improve on.

Recommend what the teacher can do to improve, this could benefit both parties in the end.

8  Tips for Providing Effective Feedback

  • Learn to control your emotions. It’s best that you do not get carried away. If you feel outraged or extremely happy, best not to let it show to keep the feedback strictly professional.
  • Do not compare the person to others. This might give the opposite effect in which the person being given feedback will feel like being put down rather than aiming higher.
  • Be specific on what you want the person to improve on. Make it very clear as to what that person’s strengths and weaknesses are.
  • Make sure that your feedback is timely. So if you’re going to give feedback on something that recently happened, make sure to do it right away instead of waiting for a long duration of time. This also helps by letting the person know that he or she is being actively monitored.
  • Try to keep the atmosphere positive. If the atmosphere is too depressing, then that person might be too scared to ask important questions such as what the can do to improve.
  • Hear out what the person has to say. This gives you their side of the story and help you understand them better.
  • Set out a goal in which these people should aim for. It allows them the opportunity to work for something, giving them opportunities to improve.
  • After a duration of time, it’s best to follow up by seeing if the person was able to improve. Which you will then give proper feedback on.

These tips are also very useful for when you need to use faculty feedback forms to assess teachers of a faculty, or lesson feedback forms to figure out if certain lessons were able to provide you with ample knowledge or not.

In the event that you need to print out these forms to help you give proper feedback, then there are feedback forms in PDF for you to view and choose from.
