What is Training Feedback Form?

How to Write a Feedback Form

Feedback forms are mostly used by companies that recently started. Since the management is still on the observation and determination of what the target market want and need, the use of feedback form is essential.  In writing a feedback form, here are some of the things that you need to know:

  • feedback forms are implemented in order to identify what areas need improvements
  • feedback forms should have enough spaces provided for answers or comments
  • these forms doesn’t have to be long; perhaps make it short enough for the relevant information to be included
  • these forms should be implemented for the welfare of both the trainee and the company (which includes the trainer and the training program)
  • questions in feedback forms should not be compulsory for the participants to answer
  • questions or evaluations should be made comprehensively
  • the questions should be relevant to the program conducted
  • allow the participant to rate your services or performance
