- Employee must fill out the Pull Notice Application Form so that the employer can acquire the employee’s driver records.
- Once the employee has finished filling out the form, the employer must verify the information such as seeing if the license number and name within the form matches the employee’s driver’s license
- The department manager/supervisor must fill out the “department” section of the form
- The employee’s driver’s license must be photocopied
- Form is then to be submitted to the DMV’s HR and service center to be processed.
