A student-athlete form is used by educational institutions for collecting information from their students who are engaging or will be engaging in various sports activities. With a student-athlete form, the institution will be able to have an updated database of their athletes, not only for attendance and personal information purposes but also for helping the medical team of the institution in determining what each athlete needs to condition himself prior to the activities lined up for him. Moreover, there is also a specific type of student-athlete form which is intended to be sent out to the parents or the legal guardian of each athlete in order to obtain their consent regarding the participation of their child in the institution’s athletics or sports activities.
Why Is a Student-Athlete Form Important?
A student-athlete form is important because it is a standardized document which allows the institution to gather the same types of information from their athletes, regardless of the sport or the activity being participated by each student-athlete. Another purpose of having the form is to determine the suitability of the student in participating in his choice of sport based on his medical examination results and the feedback of people who are assigned to train him. Additionally, the right type of student-athlete form will help the institution filter and select the athletes that they prefer to join in the games or in sports events who will be representing the name of the institution.
Types of Student-Athlete Forms
The different types of student-athlete forms vary on the pieces of information collected in each type. Below are some of the common types of student-athlete forms that educational institutions often use:
New Student-Athlete Checklist Form – If a student is a new athlete in an institution, then he will be required to submit specific requirements or documents that will validate his association with the institution and his capabilities in being an athlete. These requirements will be enlisted in a new athlete checklist form which can list the items needed to be provided immediately such as the official transcript of records of the student or his school records as well as his complete student application or admission sheet. This form does not have to fill out by the student-athlete or the applying athlete, rather by authorized personnel who is assigned to check the documents given by the applicants. Student-Athlete Information Form – This is the most basic type of student-athlete form since it serves as the initial document handed to a student-athlete applicant. The objective of a student-athlete information form is to simply gather relevant information from the applicant such as his full name, the school year that he is attending, his home address, gender, date of birth, and his school attendance history. In addition, a student-athlete information form can collect the emergency contact information details of the applicant which should have the name of his emergency contact person, the type of relationship that they have, the person’s emergency address, and the daytime and personal phone numbers of the emergency contact person. Insurance information and emergency treatment information may also be added on the form to gather more details from the applicant.

Student-Athlete Medical Information and Consent Form – The focus of a student-athlete medical information and consent form is to collect the medical details of the applying athlete. In this form, there are four sections which will have to completed by the student. The first section of the form is intended for the general information of the student-athlete ranging from his name up to his contact numbers. The second section of the form is where the medical information of the student will be disclosed. This section will have the name of the student’s family physician and his contact number along with the address and the hospital of preference of the student in events of emergencies. The student’s insurance information will be the center of the third section of the form which will state the name of the insurance subscriber, group number, policy number, insurance company, and the student’s pre-existing medical conditions. The last section of the form is intended for a medical authorization statement and a signature block for the legal guardian of the student.

Student-Athlete Pre-Participation Examination Form – This form comprises of three important contents which should be fulfilled by the student-athlete applicant. The first is the pre-participation questionnaire wherein the athlete applicant can state his details including his family history. The second is a pre-participation physical examination which is focused on gathering the general medical data of the student along with his musculoskeletal exam results, and his cardiovascular screening findings. In addition, an information sheet regarding the usage of supplements and drugs for the athletes is included since it is the welfare of the students that any educational institution assures especially when it comes to sports where the health of participants can be at stake.
Student-Athlete Pre-Participation Examination Form
Student-Athlete Statement Form – Acknowledging the credibility and eligibility of an athlete is the main purpose of a student-athlete statement form. This document will have the name of the student-athlete, the type of sport that he will be participating in, which will then be followed by three parts. In the first part, statements concerning the eligibility of the student-athlete will be enlisted such as the affirmation of the student that he was able to receive a copy of the rules and regulations of the institution, and the confirmation of the student that he understood the terms in each statement. The second part is known as the amendment consent section which is important for documenting the agreement and approval of the student-athlete to disclose his education records for the purpose of his participation in the institution’s sports events. The test whether it turned out positive or negative will be indicated along with the date of when the test was executed, where the test was conducted, and what type of substance was found by the examiners from the samples of the student-athlete.

How to Create a Simple Student Athlete Form
Creating a student athlete form is easy if the institution will only be needing the basics or the essentials of their student athlete applicant. To create this form, the steps below must be followed:
Step 1: Prepare the empty sheet for the form. Open a text processor or a software application which is intended for text and document creation. The margin and the size of the form must be customized based on the preference of the educational institution. Two of the most common sizes are in A4 and in A3 sizes. The format to be used by the end-user of the form must be identified as well, whether the user will be using a form in “.doc” or in “.pdf” and other known document formats.
Step 2: Decide on a layout and design. It is important that school forms or student forms will be kept simple and organized which is why planning the layout and the design should be executed. The layout for each section or for each portion of the form must be finalized along with the preferred typefaces to use for the contents.
Step 3: Make the header of the form. The header should have the name of the educational institution, the name of the department who is handling the application of the student-athletes, the title of the document, and the assigned document-record identification number.
Step 4: Add fields for the information of the athlete. The pieces of information which will be collected from the athlete will be the body of the form. Each data field must have a placeholder in order for it to be properly aligned and arranged resulting in an organized document. Some of the basic data and information to add on the form would be the name of the athlete, his grade level, his interests and skills, as well as his contact information.
Step 5: Include a signature block at the last portion of the form. A signature block is where the athlete or the applicant will be able to affix his signature to serve as an indication that he had validated and verified the claims disclosed in the form. A date area should also be included beside the signature area of the block which will be for indicating the date of when the form is signed.
There can be other contents and sections which can be added to the student athlete form, however, any educational institution and organization using the form must ensure the safety and the security of the information disclosed by their students or athletes. The data of each athlete must be protected, be kept confidential at all times, and should only be handed to another party if the athlete had been informed and have given his consent to authorize the use of his information.
