Student-Athlete Statement Form – Acknowledging the credibility and eligibility of an athlete is the main purpose of a student-athlete statement form. How to Create a Simple Student Athlete Form

Creating a student athlete form is easy if the institution will only be needing the basics or the essentials of their student athlete applicant. To create this form, the steps below must be followed:

Step 1: Prepare the empty sheet for the form. Open a text processor or a software application which is intended for text and document creation. The margin and the size of the form must be customized based on the preference of the educational institution. Two of the most common sizes are in A4 and in A3 sizes. The format to be used by the end-user of the form must be identified as well, whether the user will be using a form in “.doc” or in “.pdf” and other known document formats.

Step 2: Decide on a layout and design. It is important that school forms or student forms will be kept simple and organized which is why planning the layout and the design should be executed. The layout for each section or for each portion of the form must be finalized along with the preferred typefaces to use for the contents.

Step 3: Make the header of the form. The header should have the name of the educational institution, the name of the department who is handling the application of the student-athletes, the title of the document, and the assigned document-record identification number.
