What is The Role of a Beneficiary?

When an individual applies for an insurance, he or she is asked to fill up an insurance form that should contain all necessary information that are required in order to be completed. In filling up an insurance form, the applicant is asked for his/her primary and secondary beneficiaries. A beneficiary pertains to the person to whom an amount should be received from the insurance when the applicant or the insured passes away.

What is a Beneficiary Release?

A beneficiary of life insurance policy may or may not file a beneficiary release form. A beneficiary release is a statement that authorizes a medical personnel and anyone involved to release some or even all information that are essential or needed for some legal purposes.

The authorization is written in a beneficiary release form which is proven with the presence of a notary public, and the signature of the beneficiary upon submitting the form. In the release form, the beneficiary may choose or limit the people to whom he or she would like to release the information to.

The year of validity should also be included to indicate how long will the information should be released and until when it should be. Upon signing the form of both parties, the date is also indicated in order to keep track of when this agreement or release is effective.

Similar to a beneficiary release form is a medical release form, in which an individual signs if he or she permits the health personnel to release all information about the tests that were performed in treating his or her medical condition.
