How to Create a New Customer Request Form
A new customer request form is another type of customer request form which can be used by companies, however, this form is only suitable to be used by new or unregistered customers. To create this type of form, the steps below must be followed:
Step 1: Add the name of the company and the title of the form.
Both the name and the title must be placed at the topmost center part of the form or at the header. The name of the company should be stated first then the title must be indicated below it. Moreover, an area for stating the date when the form was used can also be added below the title.
Step 2: Allocate a customer information section.
This section of the form must collect the full name of the customer along with his address, contact number, transaction type, and transaction date.
Step 3: Place a text box or area for the details of the request.
Sets of questions can also be added into this section, however, the questions should acquire relevant details about the incident, the transaction, and the requests of the customer. Moreover, an area for the fulfilled actions and the suggestions of the company or employee who met the request of the customer can also be incorporated into this section.
Step 4: Include a signature block.
The signature block must have areas for both the customer and the employee or the authorized personnel who catered to the customer’s requests and needs.
Step 5: Save the form.
The form can either be saved in its default format or be converted into a different format that will allow the company to offer different methods of filing requests for its customers. The two most common formats to consider would be the “.DOC” and the “.PDF” format.
