What Is a Course Registration Form?
A course registration form is a type of document which will collect every relevant information and details of an individual who will be enrolling and be registering the course that an institution offers. Types of Course Registration Forms
Course Audit Registration Form – Course auditing allows students to participate and join classes in a course, yet their names will not be part of the roster or the list of official students since they will only be in the class for the sole purpose of self-enrichment and exploring course options. Most institutions permit students to audit a course without paying any fees throughout the period of the semester or the class. Furthermore, in order for a student to be registered as one of those who will take up a course to audit, a course audit registration form must be filled out.

Four sections comprise a course audit registration form. The first section is allotted for the name, residential address, email address, date of birth, and gender of the student while the second section is for indicating the disciplinary action that the student had acquired before in his previous course or school. The third section, on the other hand, is intended for the details of the course, and the last section is to be signed by the instructor of the course or program to be attended by the student.
Course Participant Registration Form – Companies who offer training programs and academic opportunities to their employees must prepare this type of course registration form. Just like the aforementioned course audit registration form, this form type also has four sections to be completed. However, the one who will fill out the form is the assigned registering and enrollee committee and not the registrants themselves. In the form, the name, location, and date of the course program or employee training will be stated along with the contact information of the enrollee, the details of the invoice for the payments of the course, and the list of the participants of the course.

Noncredit Course Registration Form – This is another type of course registration form which will be used by students who will only be attending the course’s classes yet will not be receiving any credits in their records after accomplishing the course. But, the difference between a noncredit course from an audited course is that a noncredit course will require the student to pay fees and avail payment discounts while the latter does not. Nevertheless, in a noncredit course registration form, the student must be able to disclose his name, mailing address, daytime phone number, email address, and signature in the allotted areas of the form.

There is also a section in a noncredit course registration form which will be for indicating the name of the noncredit course along with its starting date, fees, discounts, and total payment to be paid by the student. Additionally, a payment information section must also be filled out by the student which will be for specifying the type of payment method he chooses to fulfill his financial obligation in order to register to the course.
Rider Course Registration Form – This form is used by companies who will be providing passes to their members and personnel to operate their personal motorcycles inside the premises of the company. The form is intended to be filled out by both the registering motorcycle operator and the company’s representative who will determine whether or not the registration will be approved. Only the first and second sections of the form will have to be filled out by the operator which will be for his basic data, identification number, and the details of his motorcycle. The succeeding sections are to be completed by the company’s representative for the approval, justification or the reason for the approval or disapproval, and for indicating the types of legal forms to be submitted by the motorcycle operator along with the registration form.

Essential Data to Collect in a Course Registration Form
Although there are several types of course registration forms, each type or variety should have the following elements or contents in order for the form to be effective, efficient, and to meet its objective which is to gather data from the registrant:
- The registrant’s information: Aside from the course registrant’s full name, address, and contact details, the form must also collect the registrant’s identification number, as well as the previous courses and programs that the registrant had attended.
- The description of the course: This is where the course’s unit number, title, and prerequisite course information will be stated along with the tuition fee or payment which must be provided by the registrant to avail the course. In addition, the name of the instructor of each course and the semester when the registrant will be taking up the course can also be documented in this section of the form.
