School registration forms are documents which are intended to collect the details of a school registrant such as a student or an activity participant. Childcare School Registration Form – This form has four sections to be completed by the registrant. The name and address of the school will be stated in each page of the form to ensure that the pages will be identified if ever it will not remain intact during the registration process.

To use a school program registration form, the assigned representative or focal person of the State’s education authority will have to mark the boxes which accompany each requirement to be fulfilled by the school to comply for the registration. Remarks can also be incorporated in the form. And lastly, in order to complete the form, the focal person must affix his signature and state his name and the date when he conducted the investigation and verification of the school’s registration requirements.

How to Fill Out a School Student Registration Form
A school student registration form is another type of school registration form that educational institutions can use, however, as compared to the other forms, this is the most basic and is the most used type. To completely fill out this form type, the steps below must be followed by the parents of the child or student who would like to register in school:
Step 1: Complete the first section to introduce the student.
The information to be stated in this section will be the school year when the student will be enrolling in, the name of the school or program, and the name of the student’s previous school.
Step 2: Disclose the student’s school history.
Aside from the name of the student’s previous school, the form will also be collecting the date when the student last attended the mentioned school along with his grade level and an indication of whether he was expelled before and have attended a school out of the county or province.
Step 3: Fill in the entry fields of the student information section.
The surname, first name, and the last name of the student along with his gender, date of birth, country of birth, mailing and physical addresses, and medical information will be the contents of the student information section.
Step 4: Indicate the citizenship of the student.
The reason for knowing the citizenship of the student is to ensure that the student is residing or living in his stated address legally. In addition, some schools also offer programs to those students who are from different countries and with different citizenship to aid and guide them in their stay and education in school.
Step 5: Indicate the details of the student’s other legal guardians and alternate contact persons.
Their names, contact information, addresses, and their relationship with the student will be some of the details to disclose. Moreover, an indication of whether or not the student lives with his alternate guardians should also be included in the form.
Step 6: Complete the legal education information section.
In this section, the student’s education eligibility, custody order details, aboriginal self-identification, and independent student status will be stated.
Step 7: Read and sign the declarations and provisions.
Aside from a signature, the parent of the student should also state the name of the student or child with the parent’s or the signer’s name, and the date when the signature was affixed in the form.
