What Is a Media Release Form?

A media release form is a type of form that is used to allow the distribution of certain images or videos to the public with the full consent of whoever may appear in the said image or video. This is done so as to avoid any issues with the people in the said media.

A media release form is just another term for a Video Consent Form with the only difference being that media defines both pictures and videos while a Video Consent Form is, as the name states, only provides consent for videos and not pictures.

How to Write a Media Consent Form

When distributing any form of media to the general public, it is important to remember that you must acquire the consent of the people involved with the image before being allowed to distribute the said media. So you will need a media consent form, but how do you draft it? By following these steps:

  • Begin by identifying yourself as the photographer and all of your crew members. (If you have a crew.)
  • Identify the nature of the media whether the media is rated G for general viewers or is it rated as an explicit media material.
  • Identify any individuals who have been caught in the media and have them either consent to the media being distributed or their terms for consenting to the media being released. (In most cases this means censoring the persons involved.)
  • If it is an image, then you can use a Photography Release Form as a substitute to a media consent form.
