What is a Job Appraisal Form?

A job appraisal form, also known as a staff appraisal form, is an assessment or evaluation of the employees’ performance. This is usually done annually but for other business companies, they would rather do an assessment of the employee regularly, sometimes monthly or even weekly. This is to make sure that each of their employees are in good shape with regards to their progress and productivity.

How to Fill Out a Job Appraisal Form

In filling out a job appraisal form, there are necessary information that are needed. The following are:

  • the start and end date of the appraisal period
  • the starting date of work or the hired date of the employee
  • the name of the employee, job position and department
  • the supervisor’s name, title or position
  • list of criteria, goals or objectives (such as required skills or tasks to be performed)

  • the employee’s comments and suggestions

Performance Planning and Appraisal Form

First and foremost, the performance planning and appraisal form should be based on the agreement between the employer and employee on the performance plan such as the standards and outcomes of the performance, and the fulfillment of the critical functions and accountability during the review period.

The critical functions and accountability should be brief, concise and precisely written. And in writing an employee appraisal form, vague generalizations and terms should be avoided. It is also better if quantitative terms are used whenever possible and the job description is accurately captured as one entity.

Clear statements of conditions which defines anticipated results, are units of measurements in order to accurately and objectively measure employee’s performance level for each function and significant tasks. And in order to promote a higher level of understanding and agreement, performance standards should be developed in collaboration with the respective employees.
