  • Begin by providing fields for input of basic personal information like name and position.

  • Determine the key areas which you want to assess.

  • Determine how you want to measure these competencies. Do you want the evaluator to place check marks on appropriate columns, or do you want ratings in percentages (or in stars) instead?

  • Then, you can now create tables if you want the medical appraisal form to resemble a checklist. Or you may use blank fields to indicate ratings if your organization prefers this option.

  • Repeat that step for every job competency you wish to measure.

  • Include a short comments section at the end of the form where the evaluator can write a few sentences regarding the person he/she evaluated.
