This is especially so if you are requesting a type of cosmetic surgery that could be proven deadly when handled improperly. An example of such would be a dermal filler cosmetic surgery, where an injection filled with a certain chemical will be used as a means to remove wrinkles and skin deformities. This type of cosmetic procedure would typically require you to write a consent form for dermal filling.

What Is a Dermal Filler Consent Form?

A dermal filler consent form is a tool used by most cosmetic surgeons who will handle dermal filling procedures. A dermal filling is a type of procedure where the cosmetic surgeon will be injecting a certain chemical substance into your skin to even out the wrinkles, thus, making you look younger and more balanced.

This type of consent form is needed by the cosmetic surgeon to be given to the patient or client as a means of informing the patient, that as with any surgery, there will be some form of risk involved with the procedure. This is also to identify at least some of the risks that may occur with the procedure. This type of process in the transaction can be somewhat similar to that of a business consent form.

What is the Difference Between Botox and Dermal Fillers?

You may decide that Botox and dermal fillers are somewhat similar, but that is actually an incorrect thought. This is because Botox, or more commonly called Botulinum toxin in the medical field, is a type of chemical that is synthesized from a bacteria known as Clostridium botulinum.

This chemical is commonly found in puffer fish venom, which in large enough dosages, can be proved to be fatal. Another thing about Botox, is that it can last from anywhere between three to four months before the patient has to re-administer the Botox.

Dermal Fillers uses a type of chemically manufactured substance that is used as a temporary long term solution to old wrinkled faces. This type of cosmetic is only effective for a duration of six to twelve months. The duration time lasts longer due to the fact that it is an artificial chemical and will give the body a hard time to absorb it.

Regardless of the type of cosmetic being used, as long as consent is required from the patient, and if the patient is young or underage, then a parental consent form is needed in addition to the required consent forms.
