This type of authorization form is then called a Medical Release Form.

Medical Leave Forms

A person has the legal right to make his own medical decisions for as long as he is mentally fit or mentally stable, he has the ability to make sound decisions, he has the capacity to understand or comprehend the information that has been discussed to him, and he has the ability to provide voluntary and willful consent.

One must remember that an Informed Consent is not exactly synonymous with a Consent Form, and having a Consent Form does not particularly rule out the need to provide a documentation of the consent that was given.
