Incident Report Form
An Incident Report is just as it sounds. It is a document that describes an incident or an unanticipated event that might require an investigation process and an analysis of the event that has occurred. Absence Report Form
Absence Report Forms are to be utilized when an employee is absent from work due to sickness or accidents. Most companies implement strict rules in notifying supervisors that they will not be able to report to work. This allows the manager to file an Absence Report. An Absence Report enables the company and the manager to be well aware of the expected duration of an employee’s absence, options to work from home, how to contact the employee, and any ongoing work that needs to be taken over by a colleague. Other than being able to provide options to accomplish an absent employee’s task, Absence Reports also allow the management to provide counselling and rehabilitative support.
Expense Report Form
An Expense Report is a report that tracks the expenses incurred in the process of performing necessary job functions, such as gasoline expenses, meals, parking, and lodging. Through an Expense Report, employers or managers will be able to track the expenditures of an employee together with an attached receipt to support the details listed in the form so that a manager can double-check and compare the cost indicated in the form with the cost on the receipt. Once the total expenses have been assessed and confirmed, a bookkeeper can then issue a check to reimburse the employee.
Employee Performance Evaluation
An Employee Performance Evaluation is a process wherein the management evaluates and provides feedback regarding an employee’s work performance for them to assess an employee’s strengths, weaknesses, achievements, and abilities. This allows employers to provide necessary steps for improvement or can be used as a basis to promote an employee or increase compensation packages. An Evaluation Report Form is typically used when conducting these types of evaluations.
