Since the beginning of human civilization, the world has been run by business-minded people. From early item trading to monetary payment buying, the world continually progresses with the help of the economy, capital, and profit. Businesses of various fields of interest have continuously been on the rise.
In each corner—tight and hidden or wide and accessible—of a country, city, or street, a business-minded individual registers his business and joins the ranks of other existing businesses, competing to stay in their comfort bubble or to go beyond the realm of complacency.
The Death of Certain Businesses
The financial world has always been a battlefield that requires great feats of wit and strategy. However, there are instances when some small businesses fail and stop their operations entirely for various reasons.
It might be a lack of a better marketing strategy or a shortage in the selection of better employees. Either scenario ends with the death of a business. Even big companies may succumb to this predicament if they don’t handle their business well.
How do employers and managers assess such a downfall from previous small businesses and avoid it entirely? What must employers and managers do to remedy problems when they come and prevent it from becoming an all-consuming fire? One primary way to know is to conduct frequent employee feedback.
What Is an Employee Feedback Form?
An employee feedback form is a formal semi-analytical type of business development document that serves to be an assessment or evaluation of either the employee’s or the company’s performance and service rendered. The information retrieved from this report is a basis on how the company and the employees are progressing—daily, weekly, monthly, or annually.
Three Categories of Employee Feedback Forms
There is no single category of an employee feedback form. If there is only one category, the form will not help assess the other factors of a company, and those areas might even be the reason why the company is failing at reaching maximum quality productivity.
An employee feedback form has three categories, with each requiring a specific area of priority to oversee. In certain situations, employees may summarize their overall feelings and opinions as a form of conclusion in the survey. Listed below are the three categories of an employee feedback form.
1. Employee Self-Evaluation
This type of employee feedback focuses on how the employee personally assesses his progress as an employee in the company. The questions are mostly designed as a personal reflection. The questions that have been written ask how the employee rates his emotional, physical, social, and mental development while in the company.
This feedback category has two types—performance evaluation and opinion evaluation. The first type deals with assessing one’s performance while the other deals with determining one’s mindset.
2. Manager Assessment of Employee
The most common and most used out of the three categories. This type of employee feedback form is aimed at how the manager assesses an individual employee. The questions are designed to help the manager in giving a professional assessment regarding the work productivity and work quality of an employee.
Has the employee made any form of progress? Is the employee able to reach the quota while achieving quality production? These are sample questions on this category.
3. Employee’s Comments on the Company
The last group deals with how an employee assesses and comments on the company’s environment and the company’s treatment of its workers. The questions are designed to give a critique on how the company provides service to its workers. Also, the employees are asked about their expectations of the workplace, the coworkers, and the company.
Are the authorities being unbiased when dealing with problems? Are there any complaints regarding the workplace? Is the environment constantly being kept clean? Are the instructions clear and reachable? These are three out of many other questions that deal with the assessment of the company.

How to Create an Employee Feedback Form
1. What Kind of Feedback Form?
There are three categories of feedback forms, with each having a specific area of priority. Carefully choose the category you wish to create. Know the information you want to acquire before concluding as to what kind of feedback form is necessary.
Write down questions that gauge the one who will answer the feedback form to give you the type of information that you want to acquire. Look at sample feedback forms on the Internet for reference on how questions are formulated.
3. Font Style Usage
Most feedback forms do not follow a strict font style rule. As long as the font style is readable and does not overlap any element found in the text. Better use default font styles?—?Garamond, Arial, Times New Roman, Calibri, Georgia, etc.— if you want to play it safe.
4. Company Logo
Most feedback forms do not necessarily require to have the company logo placed in the document. However, if you want to be different from the rest and make your report distinguishable from the rest, we suggest you add the company logo on the form. This will make the document more authoritative.
5. Output Evaluation
As you have assessed the employee, employer, and the company, you must now evaluate the document for any type of grammatical and contextual error. Carefully examine your questions if they are relevant to the kind of feedback survey that you are aiming to create. Do a thorough document evaluation, and you will be able to create a professionally effective feedback form.
