The questions are designed to help the manager in giving a professional assessment regarding the work productivity and work quality of an employee.

Has the employee made any form of progress? Is the employee able to reach the quota while achieving quality production? These are sample questions on this category.

3. Employee’s Comments on the Company

The last group deals with how an employee assesses and comments on the company’s environment and the company’s treatment of its workers. The questions are designed to give a critique on how the company provides service to its workers. Also, the employees are asked about their expectations of the workplace, the coworkers, and the company.

Are the authorities being unbiased when dealing with problems? Are there any complaints regarding the workplace? Is the environment constantly being kept clean? Are the instructions clear and reachable? These are three out of many other questions that deal with the assessment of the company.

How to Create an Employee Feedback Form

1. What Kind of Feedback Form?

3. Font Style Usage
