One way to ensure that a business and employment company will grow and expand is to hire the right individuals. However, after the hiring process or during the period of employment, the company must ensure that its employees are still providing high-quality service. In order to gather observations and make resolutions, and employee evaluation program must be conducted by the company.
What Is an Employee Evaluation Form?
An employee evaluation form is the type of document to use by companies for an employee evaluation program. The form will record the details of the evaluations or observations of the evaluator about an employee in the company. With the form, both the employee and the evaluator will be able to identify problems or areas of concern in the employee’s performance, as well as create plans which will address the problems effectively throughout the period of the employee’s employment.

In the second section, the general criteria which will have to be scored and rated by the evaluator will be enlisted with a rating scale and areas for both the evaluator’s comments and observations about the employee’s major strengths and weaknesses. The third section, on the other hand, is for the position-related criteria in lieu of the employee’s job title and what he was able to provide as part of his roles and obligations as an employee. The goals which had been set out for the employee and the recommendations of the evaluator will also have to be stated in the second section. And in order to complete the form, the evaluator must affix his signature in the allotted areas of the form and have the employee sign the form as well, and indicate the scheduled date for the next employee evaluation period.
New Employee Performance Evaluation Form – New employees need more guidance as compared to the regular employees since they are still in the process of adjusting in the workplace and meeting the company’s needs which can lead to proving themselves to be worthy of their positions in the company. Specifically, a new employee performance evaluation form will have to be used by an evaluator of a newly hired employee. In the form, the evaluator will be able to rate the employee’s work ethics, communication skills, and excellence in catering to the values of the company. Moreover, there is also a section which is intended to document the planned goals and the method or ways of how the new employee’s performance and success will be scored after the evaluation period.

Goals to Keep in Mind When Using Employee Evaluation Forms
Employee evaluation forms should be used with the right goals in mind, and not to pull down or cause a conflict in the employee’s employment. Below are some of the goals that evaluators must note when filling out any type of employee evaluation forms:
Focus on the criteria of the company
It is essential that the contents and evaluations stated in the form are based on the criteria and requirements of the company in order to avoid hitting below the belt or stating personal and unprofessional evaluations.
Present solutions which are practical
The solutions should be inline and limited to the employee’s job description and role. In addition, a timeframe to guide the employee to reach the goals and solutions can also be included in the evaluation form to further present the plans of the company.
Use plain words or explain the details to the employee
Jargons, unless used by the employee in his daily job, should be avoided at all cost. On the other hand, phrases should also be kept short, yet accurate and must be explained to the employee during the presentation of the evaluation form and the evaluation results.

Employee Evaluation Form Tips
For evaluators, when using an employee evaluation form, it is important to readily have a back up for any claims and for the evaluations and comments to be disclosed in the form. The back up should be valid documents which will prove or show the employee’s performance and abilities in certain criteria set out to be evaluated. For instance, indicating a low score or evaluation in the employee’s attendance should be backed up with the employee’s attendance sheet which shows the absences and tardiness of the employee.

On the other hand, employees who will be disclosing their feedback or thoughts about their own evaluation results in the evaluation form should also be professional in presenting facts, opinions, or suggestions. And if ever there is a concern which must be raised, then the employee must indicate in the evaluation form that he will be filing an appeal or will be requesting to schedule a meeting with the evaluator and a witness.
