It is essential that the contents and evaluations stated in the form are based on the criteria and requirements of the company in order to avoid hitting below the belt or stating personal and unprofessional evaluations.
Present solutions which are practical
The solutions should be inline and limited to the employee’s job description and role. In addition, a timeframe to guide the employee to reach the goals and solutions can also be included in the evaluation form to further present the plans of the company.
Use plain words or explain the details to the employee
Jargons, unless used by the employee in his daily job, should be avoided at all cost. On the other hand, phrases should also be kept short, yet accurate and must be explained to the employee during the presentation of the evaluation form and the evaluation results.

For evaluators, when using an employee evaluation form, it is important to readily have a back up for any claims and for the evaluations and comments to be disclosed in the form.
