What’s Included in a Durable Power of Attorney?
- State your name and the name of the agent or appointee.
- Indicate the effective date. For a Financial Durable Power of Attorney, you may choose for the form to be effective upon signing it or when you become incapacitated.
- State the extent and limitations of the powers of the agent or appointee.
- Include any special instructions and additional provisions, if any.
- Affix your signature and those of the witnesses and the notary to make it official.
