An affidavit form is a legal document with details which are sworn to be true. The authority or notary public has to sign the form in order to make it legal. The form when signed by the authority and the person writing the statement, the form becomes a legal document and is called an affidavit form. The written statements should be facts otherwise, legal action can be taken on the person.

What is a written affidavit?

A written affidavit form is an oath which is sworn in written form before a notary public or an authority. The statement written in the form are testified as facts by the affiant by signing in front of the notary public. The notary public attests the form by signing which then becomes an affidavit form which is legally accepted. The statement written by the person has to be facts, which found to be false come under the penalty of prejudice.

Affidavit of Birth

An affidavit of birth is the form which verifies the birth information of a person. If the original birth certificate of the person is lost or if it is incomplete, the affidavit of birth can be used as an alternative of the birth certificate.

Affidavit of Name Change

An affidavit of the Name change is the form which is a sworn statement written by the person stating that the changed name is official and legal. This comes to use when a name has been changed and it has to be made official. This is needed when applying for a passport, visa or a job.

Affidavit of Financial Support
